I don't think that there is a easy way of making money. It's actually part of the being a witcher thing, always being broke. Anyway, you shouldn't need to buy any equipment, just equip stuff you find and save your money for when you can craft Witcher Gear.

You do not actually lead him out, and he seems to just wait around for you. So, all you have to do is continue with the quest and find Philippa Eilhart's hideout. When you find this guy, and he gives you the crystal, go on farther through the next door leading to a platform of sorts that seems to contain the next portal taking you farther below.

Yes. My game hasn't been letting me manually save either for a while. I just assumed auto save was working but, since I died nothing loaded so I restarted, now its stuck with please wait at the start. I'm starting to get worried I lost hours of gameplay.
Story and Sword. I went with the higher one as Kevin Van Ord said to go one higher then you normally would start on in his Gamespot review. Money is REALLY hard to come by, and on the higher one you cant rest to heal. I was spending all my money on food. I hope to get a savings and go back to the higher one after a bit, but it is a little brutal.
Before this latest patch (1.04) that came yesterday they did NOT refill their gold in, i was forced to run around like crazy from merchant to merchant and sell items i didnt need, buy items i needed so the merchants got gold again so i finally had been able to sell all the crap, and in that way got my gold back...
